What Is Hantavirus?

Hantaviruses belong to a family of viruses carried by rodents. Rodents do not get sick from them, but they spread the virus in their urine, droppings, and saliva. Humans contract it when they touch or breathe in these materials.

Different types of hantaviruses exist around the globe. In the Americas, including the United States, they often cause “Hantavirus Pulmonary Syndrome (HPS)”, which mainly affects the lungs. In Europe and Asia, they usually lead to “Hemorrhagic Fever with Renal Syndrome (HFRS)”, which impacts the kidneys and can lead to bleeding issues.

The virus is named after the Hantaan River in South Korea, where it was first researched in the 1970s. Scientists now recognize many different strains, but they all spread similarly.

How Does Hantavirus Spread?

People primarily get hantavirus by breathing in small particles from infected rodent droppings, urine, or saliva. This can occur when you stir up dust in areas where rodents live, like old cabins, barns, or storage sheds.

Other ways to get it include:

  • Touching contaminated surfaces and then touching your mouth, eyes, or nose.
  • Eating food or drinking water that has rodent waste on it.
  • Getting bitten or scratched by an infected rodent, which is rare.

Most hantaviruses do not spread from person to person. The only known exception is the Andes virus in South America, which can spread through close contact in rare circumstances.

You cannot catch it from pets, insects, or other animals in the same way. It usually involves wild rodents, especially deer mice in North America.

History and Outbreaks:

Hantavirus has existed for a long time, but doctors began paying more attention in the 1990s. A significant outbreak occurred in 1993 in the Four Corners area of the southwestern United States, which includes Arizona, New Mexico, Colorado, and Utah. Healthy young people suddenly fell ill with breathing problems, leading to the discovery of the Sin Nombre virus, a major cause of HPS in the U.S.

Since then, the United States has reported around 890 cases of hantavirus disease as of the end of 2023. Approximately one-third to half of these cases have been fatal. Cases are more common in western states where deer mice live.

Globally, thousands of cases occur each year, mostly HFRS in Asia and Europe. China frequently reports many cases, but the numbers have decreased due to better rodent control.

Recent news sometimes highlights small clusters, such as on ships or in specific areas, but large outbreaks are rare because the virus requires close contact with rodents.

Symptoms of Hantavirus:

Symptoms typically appear one to five weeks after exposure. Early signs resemble the flu, which can make it easy to overlook.

Early symptoms (first 3-5 days):

  • Fever and chills
  • Fatigue and weakness
  • Muscle aches, particularly in the back, thighs, and hips
  • Headache
  • Dizziness
  • Nausea, vomiting, or stomach pain
  • Occasionally diarrhea

After several days, the illness can worsen significantly, depending on the type.

For HPS (lung problems):

  • Shortness of breath
  • Coughing
  • Fluid accumulating in the lungs (similar to drowning from within)
  • Low blood pressure
  • Fast heartbeat

This stage can begin suddenly and become life-threatening quickly. Many people may need immediate hospital care.

For HFRS (kidney problems):

  • Bleeding issues
  • Low blood pressure
  • Kidney failure (producing less urine or no urine)
  • Back pain or abdominal pain

Some individuals experience milder symptoms and recover without severe complications, while others may require intensive care.

Diagnosis and Treatment:

Doctors diagnose hantavirus using blood tests that detect signs of the virus or the body's response to it. They also inquire about rodent exposure and symptoms. Early diagnosis is crucial.

There is no specific cure or antiviral medication that effectively treats all types. Treatment usually involves supportive care in a hospital:

  • Oxygen or breathing support, sometimes with a ventilator
  • Fluids and medications to maintain stable blood pressure
  • In severe cases, ECMO (a machine that assists the heart and lungs)
  • Dialysis for kidney failure in HFRS

With prompt hospital care, many people fully recover, but the disease can be fatal without treatment. Recovery can take weeks or months.

Who Is at Risk?

Anyone who comes into contact with rodents or their droppings is at risk. Higher-risk groups include:

  • People living in rural areas
  • Hikers, campers, and those who clean cabins
  • Farmers and those working in barns or sheds
  • Individuals cleaning long-closed buildings without protection

It is more common in spring and summer when people clean or spend time outdoors, but it can occur at any time of year.

Prevention: How to Stay Safe:

To avoid hantavirus, the best approach is to keep rodents out of your home and workspace. Here are some practical tips:

Rodent-proof your home:

  • Seal gaps and holes with steel wool or caulk.
  • Store food in airtight containers.
  • Keep garbage in bins with secure lids.
  • Remove clutter where mice can hide.

When cleaning areas with signs of rodents:

  • Wear gloves, a mask (N95 if possible), and eye protection.
  • Do not sweep or vacuum dry droppings, as this spreads dust.
  • Moisten the area with disinfectant or a bleach solution (one part bleach to ten parts water), let it sit, then wipe it up.
  • Double-bag and dispose of waste properly.

Outdoors:

  • Avoid touching live or dead rodents.
  • Use traps or call professionals for infestations.
  • Air out closed cabins before entering.

If you suspect exposure and develop flu-like symptoms with breathing issues, see a doctor right away and mention your contact with rodents.
